Employee Acknowledgement Form
All new employees of Saunders Staffing Inc. or Personnel Pool of Virginia are required to read and sign confirmation of this document. By doing so, you acknowledge that you understand all the policies set forth herein. Please print copy for your records & fax signed copy to our office. FAX: 304-325-6817
I hereby authorize SAUNDERS STAFFING INC. OR PERSONNEL POOL OF VIRGINIA to process my application through standard background verifications as required by the company. I grant all individuals and organizations listed in my application or developed as a result of an investigation the right to release any requested information. Records and information obtained will be used in determining my eligibility for employment with the company.
I understand that if SAUNDERS STAFFING INC. OR PERSONNEL POOL OF VIRGINIA employs me I may be required to submit to a substance screening. The results of this screening may be used to determine my eligibility for employment. I will agree to submit to a screening when requested for cause, on a random basis or immediately following a work related accident and/or incident.
I hereby authorize any physician or medical facility retained by SAUNDERS STAFFING INC. OR PERSONNEL POOL OF VIRGINIA to release the results of a substance screening to the authorized Saunders Staffing, Inc. representative.
Furthermore, I understand that Safety is my responsibility and agree to do my part to ensure that a safe job site is maintained at all times. I agree to report all unsafe conditions to my SAUNDERS STAFFING INC. OR PERSONNEL POOL OF VIRGINIA representative.
I hereby acknowledge that I have read, understand and shall comply with the SAUNDERS STAFFING INC. OR PERSONNEL POOL OF VIRGINIA policies identified.
